Getting a Physical Therapy Referral
In the state of Florida, a physical therapy referral from a physician is not required. You may self-refer to physical therapy for up to 30 days, depending on your insurance plan. If continued physical therapy is needed, your physical therapist can help attain a referral from your physician.
Referrals from physician offices can be faxed to 904-279-0963.
Our front office staff will gladly verify all of your insurance benefits for you relating to physical therapy and let you know the information prior to your first appointment.
